Mental Health Self Administered Instrument

The PHQ9 is self-administered instrument that assesses mental health status, scoring each of the 9 DSM-IV criteria as “0” (Not at all) to “3” (Nearly every day) as given below (Spitzer et.al., 1999). Your participation is voluntary, you may take your own decision to take part by ticking the appropriate response buttons, that would help you in understanding your need for mental health help.

We assure you that all information shared here will not be shared with others. Confidentiality and anonymity of this service is assured. However, there are certain limitations to confidentiality during instances: 1. of harm to self/others; 2. when there are indications of abuse of any form.
